Why this distinction exists at all
Methylene blue was first synthesized in 1876 as a textile dye. That is not trivia, it is the reason the purity question is so sharp in this category and not in others.
Most supplements are agricultural. Someone grows a plant, extracts it, and encapsulates it. Methylene blue is synthetic and has been manufactured at industrial scale for nearly 150 years, almost none of it intended for human consumption. How to read a Certificate of Analysis
A COA is a lab document showing what a specific batch was tested for and what came back. Here is what to actually look at.
1. Is it batch-specific? The COA should reference a lot or batch number. A generic document with no batch reference may be a raw-material supplier's paperwork rather than a test of the finished product.
2. Is it dated? Look for a test date within roughly the last twelve months. Older documents may not reflect current sourcing.
3. Who ran the test? An independent, accredited laboratory, not the manufacturer's internal QC.
4. What is the assay result? This is the purity figure, and it should be a number. "Ultra pure" and "premium quality" are adjectives, not specifications.
5. Are all four heavy metals reported? Lead, arsenic, mercury, cadmium. Each with a numeric result and a specification limit. A document that reports one and stays quiet about the other three is incomplete.
6. Are residual solvents addressed? Manufacturing methylene blue involves solvents. A thorough COA reports on what remains.
7. Does it confirm identity? Identity testing verifies the material is methylene blue and matches the reference standard.
The question almost nobody asks: what about the carrier?

You consume the carrier in far greater volume than the active compound. A 1 mL serving of a 1% solution contains 10 mg of methylene blue. The remaining roughly 990 mg is whatever it is dissolved in.
